There is a section where NHS Trusts share their experiences and successful new ways of working, and another section which looks at care in the US models.The website contains information about Department of Health policy, and specialist information around nurses and practice based commissioning.There are also separate sections for the Workforce Change in Long Term Conditions Programme, which is looking at new ways of working across the NHS, and the National Primary Care Development team (NPDT) which works particularly with GPs.Finally, the website brings regular news of developments in the management of long term conditions, and news of events.You can also contact the team with your questions and contributions.
